To reduce the risks of spreading Coronavirus for patients and staff we will NOT accept paper prescription requests until further notice.  We do not want patients entering the practice exposing themselves or our staff to risk of infection. You need to decide where we should send your prescription electronically. It can be to any pharmacy. You will not be able to collect your prescription from the Reception desk - please let us know your pharmacy preference. 

In some circumstances during this pandemic we will accept telephone requests for those unable to use the alternatives above and who do not have anyone available to help them, but you will still need to have a nominated pharmacy.

Annandale Medical Centre provides a travel service to our permanently registered patients.


Our travel appointments are limited so please complete the travel questionnaire below and submit electronically or print and hand into reception.  Paper copies of the questionnaire are also available from the surgery. We cannot provide travel advice without receipt of a completed travel questionnaire.


Some vaccinations required may incur a charge. Please take time to check review the charges below prior to handing in your completed questionnaire. Some vaccinations may require more than one injection for adequate protection, so two/three appointments may be needed before you travel.


Please complete the questionnaire as fully as possible.  Once submitted your questionnaire will be passed to the Practice Nurse who will review what vaccines, if any are recommended for your destination, please contact the surgery 48 working hours after completion.
